Where's the source of the correction to the First Binet Form? I confess I can't remember where I got the original from, but it's different again from what it has on MathWorld, which has $m^2 + 4$.

I wouldn't suspect so. I looked around a bit on the Binet form, but didn't find any reference to it (my bad, it seems). Then I changed it to one of the two forms which would make the statement not contradictory to Euler-Binet Formula. Apparently I chose the wrong one; it's repaired now.
